Photo / Video Consent

I grant Envision Dance Lab permission to photograph and/or record video of my child(ren) during classes, rehearsals, performances, and studio events. I understand these images and videos may be used for promotional purposes on the studio website, social media, flyers, and other marketing materials. I can revoke this consent in writing at any time.

Liability & Participation Waiver

I understand that dance is a physical activity and that participation involves inherent risks, including but not limited to injury. I voluntarily assume all risks associated with my child(ren)'s participation. I release Envision Dance Lab, its owners, instructors, and staff from any and all liability for injuries, damages, or losses occurring during classes, rehearsals, performances, or studio-related activities, except in cases of gross negligence or willful misconduct.

Tuition Policy

I understand that tuition is due by the date specified each month or semester. Late payments may incur fees. There are no refunds or credits for missed classes. I am responsible for keeping my account in good standing and for any tuition, costume, competition, or other fees associated with my child(ren)'s enrollment.

Attendance Policy

I understand that regular attendance is important for progress and class cohesion. I will notify the studio in advance when my child will be absent. Make-up classes may be offered at the studio's discretion but are not guaranteed. Excessive absences may affect placement in performances, competitions, or advanced classes.

Dismissal Policy

I understand that Envision Dance Lab reserves the right to dismiss any student for inappropriate behavior, unsafe conduct, disruption, non-payment, or violation of studio policies. Dismissed students are not entitled to refunds. The studio also reserves the right to remove a student from a class if they are not emotionally or physically ready for the level of instruction.

Uniform Policy

I understand that students must purchase and wear the appropriate dance uniform based on the dancer's age group for all classes. Uniforms help create a focused, safe, and inclusive environment. I am responsible for ordering the correct uniform bundle or attire for each registered dancer before classes begin.
